Daycare Receipt Template
A daycare receipt confirms childcare payments for a set period. It needs the provider name, address and tax ID, the parent name, each child’s name, the dates covered, the amount paid and the balance. Parents need it to claim the child care tax credit or to be reimbursed from a dependent care FSA. You can make one free below.

This is the receipt parents chase every January. Without the provider tax number on it, a childcare claim usually cannot be filed at all, which is why so many requests arrive months after the care happened.
What goes on a daycare receipt
- Provider name, business name if different, address and phone number
- Provider tax identification number, either an EIN or an SSN
- Licence number where your state requires one
- Parent or guardian name
- Each child’s name, and date of birth if the provider records it
- The period covered, written as a date range rather than as a month name
- Rate and how it is charged: weekly, daily, hourly or per session
- Amount paid, payment method and date received
- Any balance still outstanding
- Provider signature

A year-end statement like this is usually more useful to a parent than twelve separate weekly slips, because the tax credit is claimed on the annual total.
Why the tax number matters
In the US, claiming the Child and Dependent Care Credit means completing Form 2441, and that form asks for the provider identification number. Dependent care FSA administrators ask for the same thing. A receipt without it will come back. If you are a home-based provider and you do not have an EIN, you can apply for one free from the IRS rather than putting your Social Security number on every family’s paperwork.
Rules differ outside the US, but the pattern holds: the authority wants to be able to identify the provider being paid.
