Medical Receipt Template
A card statement showing only a clinic name proves nothing to an insurer or an HSA or FSA administrator. A proper medical receipt shows the provider, patient, date of service, what was done, the charge and the amount paid. Make one free below with no signup, and download it as a PDF.

Patients ask for this more than practices expect, because a card statement showing a clinic name proves nothing to an insurer or a benefits administrator. They want to see what the money bought.
What goes on a medical receipt
- Practice or provider name, address and phone
- Rendering provider name, credentials and NPI number
- Practice tax identification number
- Patient name and date of birth
- Date of service, and place of service
- Each service on its own line with a CPT or HCPCS procedure code and a plain description
- Diagnosis code linked to each service, usually ICD-10
- Charge per line, and the total charged
- Amount paid by the patient, the method, and the date received
- Insurance payment or adjustment if any, and the remaining balance
One visit, coded
Office visit, established patient, 25 minutes. CPT 99214, diagnosis M54.5, charged $215. Therapeutic exercise, CPT 97110, one unit, $58. Total charged $273. Patient paid $273 in full by card on the day. Provider NPI 1548392017, practice EIN 45-6677881, date of service 8 April, place of service 11.
Superbill or receipt
These overlap and get confused. A superbill is prepared so a patient can submit it to their insurer for reimbursement, and it must carry the diagnosis and procedure codes the insurer needs. A receipt is narrower: it proves payment. In practice most patients paying out of pocket want a document that does both, which is why the codes belong on it. If you need the full insurer facing version, use the medical invoice and superbill template instead.
For an HSA or FSA claim, the administrator is looking for the date of service, the patient, a description of the care and the amount paid. A receipt without a date of service is the most common reason those claims are rejected.

Medical receipt, medical bill, superbill or EOB: what is the difference?
| Document | Who issues it | What it is for |
|---|---|---|
| Medical receipt | The provider | Proves the patient paid, and for what |
| Medical bill or statement | The provider | Asks for payment and lists charges, insurance payments and the balance |
| Superbill | The provider | Gives the patient the codes needed to claim from an out-of-network insurer |
| Explanation of benefits (EOB) | The insurer | Explains what the plan paid and what the patient owes; it is not a bill |
A second example: a self-pay clinic visit
Receipt 0318, date of service 12 October 2026, place of service: walk-in clinic. Patient: Sam Rivera, date of birth 4 March 1988. Service: urgent care visit, charged $140. Throat swab test, charged $35. Total charged $175. Patient paid $175 in full by card on the day. Provider and tax ID shown at the top. Procedure and diagnosis codes are entered by the clinic from its own billing system.
Who should fill in the diagnosis and procedure codes?
The provider or their billing staff. Patients should never guess a code, because a wrong or invented code can get a claim denied. If you need coded paperwork to claim from an insurer, ask the practice’s billing office for a superbill or an itemised statement, and use a receipt like this one to show the payment.
What records should patients keep?
- Itemised receipts that show the date of service, provider and amount paid.
- Explanation of benefits statements from the insurer.
- Prescription receipts from the pharmacy. See the pharmacy receipt template.
- Dental receipts. See the dental receipt template.
- A note of any referral or pre-authorisation reference.
Keep them as long as you may need to appeal a claim, support an HSA or FSA withdrawal or prepare a tax return. Ask your plan administrator or an accountant how long that is for you.
How should you handle the privacy of a medical receipt?
A medical receipt shows health information, so share it only with the people who need it, such as your insurer, your benefits administrator or your accountant. If you run a healthcare practice, your own privacy duties apply, for example HIPAA in the United States. A payment record does not make a practice compliant on its own, so check your obligations. The generator on this page runs in your browser, so what you type is not uploaded to our servers.
Which medical receipt mistakes cause claims to fail?
- No date of service, only the payment date.
- A card slip or balance statement instead of an itemised receipt.
- The patient name does not match the name on the insurance card.
- No provider name, address or tax ID.
- Services described only as consultation, with no itemised line.
- Missing codes on a claim that needs them.
- Handwriting that cannot be read.
You may also need the medical invoice and superbill template or the itemized receipt template. For a simpler layout, use the general receipt template, and read invoice vs receipt if you are unsure which document you need.
Frequently asked questions
What does an insurer need on a medical receipt?
The provider name and NPI, the practice tax ID, the patient details, the date of service, and each service listed with a CPT procedure code and an ICD diagnosis code. Without the codes an out-of-network claim usually cannot be processed.
Is a medical receipt enough for an HSA or FSA claim?
Usually, provided it shows the date of service, who was treated, what the care was and the amount paid. A card slip or a balance forward statement is normally rejected because it lacks the date of service.
What is the difference between a superbill and a receipt?
A superbill is built for the patient to submit to an insurer and carries full diagnosis and procedure coding. A receipt proves payment was made. Many practices issue one document that serves both purposes.
What is a doctor’s receipt?
A doctor’s receipt is a document a medical practice gives a patient as proof of payment for a visit or treatment. It shows the provider, the patient, the date of service, the services and the amount paid.
How do I create a medical receipt?
Enter the provider details, the patient name, the date of service, each service with its charge, the amount paid and the payment method. Add procedure and diagnosis codes only if the provider supplies them, then download the PDF.
What is the format of a medical bill or receipt?
A header with the provider details and tax ID, the patient details, the date of service, an itemised list of services with charges, the payments received and any balance remaining.
Is a medical receipt the same as an EOB?
No. An EOB is issued by the insurer to explain what the plan paid. A receipt is issued by the provider to show what the patient paid.
Can I get a receipt after the visit?
Yes. Ask the practice’s billing office for an itemised receipt or statement. Most can provide one by email or through a patient portal.
