Toll Receipt Template

A toll receipt records a road, bridge or tunnel charge. It shows the operator, the entry and exit points, the date and time, the vehicle plate or transponder, the axle class, and the amount charged. Drivers claiming travel costs need the route on it, because a bare amount does not show which journey it belonged to. You can make one free below.

Tolls are almost never paid at a booth any more, which is the problem. The charge lands on a statement weeks later as a line reading the operator name and a figure, and by then nobody remembers which trip it was.

What goes on a toll receipt

  • Operator or authority name, and the road, bridge or tunnel
  • Transaction or reference number
  • Date and time of the crossing
  • Entry plaza or point, and exit plaza or point
  • Lane, and whether it was a cash, tag or plate read
  • Vehicle registration plate
  • Transponder or tag number, and the account it billed
  • Axle or vehicle class, since rates step up by class
  • Toll amount, and the posted rate if a tag discount applied
  • Any administration fee for a plate read rather than a tag
  • Account balance after the charge, on a prepaid tag

Two things matter there. The axle class explains the rate to anyone checking, and the tag discount explains why the charge is lower than the sign said.

Toll charges and violation notices

A toll receipt and a violation notice are different documents and should never be mixed. A receipt records a charge paid at the correct rate. A violation notice is issued when a crossing was not paid, and it carries a penalty on top of the toll. If the toll was later settled, the paperwork should show the original toll and the penalty as separate lines, because most employers will reimburse the toll and refuse the penalty. Keep them apart, or one line can sink the whole claim.
